2010 "How Work Flows" Conference Registration

Thank you for your interest in our 2010 user conference, How Work Flows. Please fill out the following form in order to register. If you have a docusphere.com login you may want to sign in first as certain fields will default from your user id, but it is not necessary. All fields marked with an asterisk (*) are mandatory. You will receive an email confirmation when we have received your registration.

All personal information will be held in the strictest of confidence and will not be released to any thrid parties unless you specifically authorize the release.

Please let us know if you have any special dietary requirements (e.g. vegetarian).

General Conference Registration

Please select all that apply. While there is no charge for this part of the conference we will use this information to determine the appropriate seating and meal requirements.

Billing Information

If you have selected to take an optional class, please select the preferred authorization method. If a PO or letter of authorization is required for payment, you may attach an electronic copy to this registration (via file attachments at the end of this form). If authorization is required but not attached, someone will contact you shortly in regards to those details.

IIS will send an invoice to the billing address you supply below once your authorization or PO has been received (if required). Please be sure to include the name of the individual, dept, or mail stop if required. All classes must be piad for in full at least two weeks prior to the start of the conference.
